Central and Eastern Sydney Primary Health Network is proud to run the Central and Eastern Sydney Primary Health Network Primary Health Awards 2026. The awards recognise primary health professionals and organisations that have provided exceptional service or an outstanding contribution to primary health care in the previous financial year

 

Three nominees will be shortlisted for each category and invited to attend the awards ceremony on 15 October at The Revy @ Jones Bay Wharf, Pyrmont. 

 

Complete the below form if you would like to nominate yourself or your organisation in one of the four award categories. 

  

Nominations close at midnight on Friday 14 August.   

 

Please note that EIS Health Board members and members of CESPHN's Clinical and Community Councils are ineligible to apply.

Award criteria

Please note the criteria that nominations will be scored against:

  • The service is located in the CESPHN area
  • The application described an initiative that has provided an exceptional service or program, a quality improvement activity, or shows outstanding contribution to primary health care 
  • The application addressed the needs of individuals or communities experiencing significant health or social disadvantage
  • The application demonstrated measurable improvements in service quality and care
  • The activity must have taken place within the 2025/26 financial year (but may also extend across multiple years).






Excellence in Aboriginal health and wellbeing achievement - Reason for nomination

Open to Aboriginal and Torres Strait Islander people, organisations or anyone who works in primary health or community services who has made a contribution to excellence in Aboriginal health and wellbeing. Please consider the following questions in your response:


How did the nominee demonstrate respect, build sustained and inclusive relationships and/or create opportunities for Aboriginal and/or Torres Strait Islander communities or individuals?

How did the nominee’s activity make a difference to reconciliation and healing in a culturally competent and safe manner?

What is the nominee’s connection to the CESPHN region?
